New Student Registration

All new students entering Martensdale-St.Marys Community School District must start
their registration process in the Superintendent’s Office.

All new students must provide the proper documentation to establish residency.  The documents provided must always state the student’s physical address.  A post office box in not an acceptable address for the purpose of determining residency.

You must have proof of residency in order to enroll your child.  If you do not know whether your residence is within the Martensdale-St. Marys Community School District, contact Jill Gavin, Business Manager at 641.764.2608 or go to the Warren County Assessor site: http://beacon.schneidercorp.com/?site=WarrenCountyIA

Pay school fees or complete a fee waiver and free/reduced lunch application.  See Karen Tomlinson in the Superintendent’s Office for more information.

Bring any special needs documents if applicable.  (Example:  Individualized Education Plan / IEP, 504 Plan, Gifted and Talented / Enrichment Plan, Individualized Health Plan (IHP), etc.)

Name, address and phone number of your child(ren)’s previous school.

You will need to complete the following forms:

1. New Student Registration Information
2. Student Health Update
3. Athletic Physical (7-12)
4. Student Permission Information
5. Home Language Survey
6. Dental Screening (kindergarten, 9th grade and all out-of-district transfer students)
7. Athletic Medical & Transportation Eligibility (7-12)
8. Student Driving Permit Form (7-12)

You will need your child’s up to date immunization record, dental screening, lead screening (preschool and kindergarten only) and proof of birth; i.e., a birth certificate.

Once you have gathered and completed the requirements above, please call 641.764.2470, x516 for Elementary or email brenda_depauw@mstm.us or call 641.764.2486, x520 for Jr./Sr. High School or email christy_henderson@mstm.us to set up an appointment.  You must have all of the documentation with you before we can enroll your child(ren) in the Martensdale-St. Marys Community School District.
